Save Time At Your First Appointment!

Before your consultation, we’ll email you a welcome letter and an online link to complete all the necessary forms. It’s super fast, and there’s no need to print anything out. This will save time at the appointment.

But don’t worry if you forget or don’t have time – we’re happy to provide an iPad when you arrive to quickly complete the forms.

Please note that we can make your visit most informative for you if you come with the following information:

• The name of your dentist and any X-rays provided to you (if applicable)
• A list of medications you or your child are presently taking
• Any dental insurance information

If desired, PDF versions of the forms are available to print and fill out before your first appointment.
